Master Forms for Trustees |

| NOTE: These are the monthly master forms to be used for each
sweeps.
Each month, you will need to transfer the information (Deadline, Green, Entry,
and Cart Fees, Competition Format, etc.) from the yearly Sweeps Schedule as
provided by the Tournament Director to the first sign up sheet and have your
club members sign up to participate. Transfer this information to the second sheet
and send with total payment to
